Which assessment is most important when a client reports severe pain?
- AThe location of the pain
- BObjective signs of pain such as tachycardia
- CThe client's description of the pain✓
- DThe intensity of the pain
The core principle of pain assessment is that 'pain is subjective,' and the client's self-report is considered the gold standard for assessment. Pain is more than a physiological sensation; it also encompasses emotional and psychological dimensions, so the client's subjective description provides the most reliable diagnostic basis. While intensity and location are parts of the assessment, without a comprehensive description (including pain quality and aggravating and relieving factors), an effective nursing plan cannot be developed. Objective signs such as tachycardia may be caused by anxiety, fever, or circulatory issues and are not specific indicators of pain; they cannot replace the client's subjective report.
