Digoxin has both positive inotropic and negative chronotropic effects. Before administration, the nurse must auscultate the apical pulse for one full minute. If the adult heart rate is less than 60 beats/min, the medication must be withheld and the provider notified to prevent serious bradycardia or fatal arrhythmias from overdose. Compared with blood pressure or potassium level (although hypokalemia increases toxicity, heart rate is the first parameter to be assessed before administration).
