In the informed consent process, the nurse's role is as a 'witness' and 'verifier.' The nurse is responsible for confirming that the client's signature is given voluntarily and that the client is mentally competent, and for verifying that the client has received adequate explanation from the physician. However, explaining the surgical risks, alternatives, and other medical details is the legal responsibility of the physician; the nurse must not provide these explanations in the physician's place, to avoid legal exposure.
