Alendronate 為雙磷酸鹽類，具極高食道刺激性。正確服藥方式為空腹、喝足量水，且飯後需直立至少 30 分鐘以防藥物逆流造成食道潰瘍。若有吞嚥困難代表可能有食道損傷，需立刻回診。絕對不可與食物同服，會嚴重影響藥物生體可用率。
Alendronate is highly irritating to the esophagus and must be taken on an empty stomach with a full glass of water to ensure rapid gastric transit. Patients should remain upright for at least 30 minutes after dosing to prevent reflux and esophageal ulceration, making bedtime administration unsafe. Any difficulty swallowing requires immediate provider notification as it may indicate esophageal erosion.
