Medication safety is central to patient safety. According to the National Patient Safety Goals (NPSG), at least two patient identifiers (such as name with date of birth or medical record number) must be used prior to administration. When asked for their name, a client may misidentify themselves due to hearing impairment, confusion, or anxiety, so reliance on a single piece of information is insufficient to ensure safety. In addition, medication verification must follow the three checks and five rights; it cannot be performed only once, nor may the verification responsibility be delegated to another person.
