A nurse is preparing to administer potassium chloride (KCl) IV piggyback to a client. Which nursing action is essential for client safety?
- AMonitor for symptoms of hyperkalemia only after the infusion
- BAdminister the dose as an IV bolus
- CAdminister the medication through an infusion pump at a rapid rate
- DEnsure the medication is diluted according to facility policy✓
Potassium chloride (KCl) is a high-alert medication. Intravenous infusion of potassium must be appropriately diluted, and direct IV push/bolus is strictly forbidden, because a high concentration of potassium directly stimulates the myocardium and causes fatal arrhythmias (such as ventricular fibrillation) or even cardiac arrest. During administration, the infusion rate must be precisely controlled using an infusion pump, and cardiac rhythm and electrolytes must be monitored throughout.
