Red man syndrome is a histamine-release reaction caused by rapid infusion of vancomycin. Common manifestations include flushing of the upper body (face, neck, chest), hypotension, tachycardia, and intense pruritus. This is an anaphylactoid (pseudoallergic) reaction rather than a true allergic reaction (such as urticaria, respiratory distress, etc.) and can usually be relieved by slowing the infusion rate or increasing the volume of diluent.
