A nurse is planning care for four clients. Which client should the nurse assess first at the beginning of the shift?
- AA client with a new diagnosis of hypertension scheduled for discharge
- BA client with chronic heart failure requesting assistance with morning hygiene
- CA client with a postoperative appendectomy complaining of sudden-onset shortness of breath✓
- DA client with type 2 diabetes who has a fasting blood glucose of 140 mg/dL空腹血糖為 140 mg/dL 的第二型糖尿病病人
After handoff, the nurse should prioritize assessment using the ABC principle (Airway, Breathing, Circulation). A postoperative client with sudden-onset shortness of breath should raise a high suspicion for a life-threatening complication such as pulmonary embolism (PE) or pneumothorax, which compromises oxygenation and life and must be addressed first. Other options, such as elevated blood glucose in a client with chronic disease or a client preparing for discharge, are relatively stable.
