Key points for chest-tube care are maintaining a closed system and gravity drainage. The system must be kept below the level of the chest to prevent backflow of fluid; connections should be taped to prevent disconnection; and fluctuation (tidaling) of the fluid in the water-seal chamber with respiration is normal and indicates patency. Stripping the tubing is prohibited because it generates high negative pressure that can damage tissue; clamping should not be performed without an order, except when changing the system or per provider instructions, as it increases the risk of tension pneumothorax.
