Potassium chloride (KCl) is a high-alert medication that is highly irritating to blood vessels. If infiltration occurs, the medication enters the subcutaneous tissue and causes severe tissue necrosis and vasoconstriction. When the nurse identifies signs of infiltration (swelling, coolness, pallor), the first priority is to stop the infusion in order to prevent additional drug from continuing to enter the tissues; subsequent management can then follow.
