The pathophysiologic basis of acute pancreatitis is premature activation of pancreatic enzymes within the pancreas, resulting in autodigestion and inflammation of the gland. The core principle of management is "pancreatic rest," which reduces pancreatic secretion, relieves pain, and prevents disease progression. Maintaining NPO status during the acute phase is therefore the most fundamental and critical order, until pain subsides, bowel motility returns, and amylase levels stabilize, after which a gradual reintroduction of diet may be initiated. Clinically, the nurse should assess the client's pain and GI function, strictly enforce the NPO order, and administer IV fluids as ordered to maintain fluid and electrolyte balance.
