Delegation follows the Five Rights of Delegation: right task, right circumstance, right person, right communication, and right supervision. Assistive personnel (AP) are appropriate to perform routine, stable, non-invasive tasks that do not require professional nursing judgment, such as measuring vital signs (A), assisting with post-mortem care (C), and providing simple physical treatments (E). Feeding a patient with dysphagia (B) carries aspiration risk and requires direct nursing assessment. Documenting the response to pain medication (D) involves professional judgment and falls within the RN's scope; it may not be delegated.
