When faced with a client experiencing hallucinations, the strategy is to 'accept reality but deny the hallucination.' Acknowledge the client's experience (the hallucination is real to them) while firmly stating the nurse's reality (I do not hear them). Option A is somewhat weak in emotional resonance; option B uses a 'why' question that can provoke defensiveness; option D shifting to medication at this moment may be perceived as hostile by the client.
