Informed consent is a legal and ethical right of the client. When the client refuses, the nurse's role is as advocate and communication bridge. The nurse should assess the reason for refusal and confirm understanding, but cannot coerce. If the client maintains refusal, the nurse must promptly notify the physician, who will re-explain risks and alternatives or assess decision-making capacity. Nurses cannot substitute for physicians in medical explanations nor force signature.
