A nurse is assisting a client with mobility limitations in transferring from bed to chair. Which action is the highest priority for client safety?
- AAsk the client to hold onto the nurse's neck
- BPlace a gait belt around the client's waist在病人腰部放置防護腰帶(gait belt)。
- CLock the wheels on both the bed and the chair✓
- DPosition the chair on the client's stronger side
During a transfer, the most common cause of falls is movement of the furniture. Ensuring that the wheels of the bed and chair are locked is the first step in preventing displacement. A transfer (gait) belt is a useful adjunct, but it is ineffective if the object itself is unstable. Having the patient grasp the nurse's neck causes cervical injury to the nurse and destabilizes the patient's center of gravity; correctly, the patient should be supported on the seat of the chair or on the nurse's shoulders. Placing furniture on the unaffected side is meant to facilitate movement, but environmental stability takes precedence over orientation.
