A nurse is preparing to administer medication. Which action represents the most effective way to prevent medication errors?
- AIdentifying the client using two unique identifiers
- BVerifying the dosage with another nurse before administration
- CScanning the client's barcode and the medication barcode✓
- DChecking the medication label against the MAR three times
The cornerstone of preventing medication errors is establishing layered safety nets. Barcode medication administration (BCMA) uses digital technology to perform real-time matching of the patient's wristband, the medication label, and the medication administration record (MAR), and is currently the tool with the strongest clinical evidence for reducing human identification errors. Although the nurse should still follow the principles of medication administration, the automated system provides a final electronic safeguard that minimizes the rate of medication errors.
