Digoxin has a positive inotropic effect (increasing myocardial contractility) and a negative chronotropic effect (slowing the heart rate). The clinical safety standard for administration in adults is generally an apical heart rate greater than 60 beats/min. If the heart rate is less than 60 beats/min, administration may precipitate severe bradycardia or arrhythmias; therefore, the nurse must withhold the medication and notify the provider to prevent drug toxicity.
