異常事件報告的目的是紀錄造成傷害或具潛在風險的事件，以便進行根因分析（RCA）。跌倒、給藥錯誤、針扎事件均屬重大病人或員工安全事件；遺漏體溫記錄屬記錄缺失，通常由主管輔導，不需異常事件報告；餐點不滿意屬服務滿意度，不屬於安全事件。
Incident reports are required for events that cause harm or pose significant safety risks, such as patient falls, medication errors, and needlestick injuries. Simple documentation omissions or service complaints do not require incident reports.
