Digoxin has a narrow therapeutic index and is highly prone to toxicity. Common signs of toxicity include arrhythmias (such as bradycardia), neurologic symptoms (visual changes such as yellow-green halos), and pronounced gastrointestinal symptoms (nausea, vomiting). In addition, hypokalemia increases myocardial sensitivity to digoxin and dramatically raises the risk of toxicity, so heart rate and serum potassium must always be verified before administering the dose.
