暫停確認（Time Out）是手術安全核對清單（Surgical Safety Checklist）的核心步驟。此程序要求在切開皮膚（Incision）前，手術室內所有人員必須停止手邊工作，共同確認病人的身分、手術部位（Site）、手術術式（Procedure）以及是否已給予預防性抗生素。此舉是為了徹底消除手術錯誤（Wrong-site, Wrong-procedure, Wrong-patient surgery）的發生機率。
The 'Time Out' is a core step of the Surgical Safety Checklist. The procedure requires everyone in the operating room to pause all other activity immediately before the incision and jointly verify the client's identity, surgical site, and procedure, as well as whether prophylactic antibiotics have been administered. The goal is to eliminate the occurrence of wrong-site, wrong-procedure, and wrong-patient surgery.
美國手術室對於 Time Out 的執行非常嚴格，若護理師發現未執行 Time Out 卻已開始手術，有權直接喊停並報告。台灣臨床雖有類似制度，但有時會流於形式，美國醫院則將其視為不可妥協的標準作業程序。
