If a patient develops paresthesia in the distal extremities and around the mouth after thyroidectomy, accidental injury to the parathyroid glands leading to hypocalcemia should be strongly suspected. Trousseau's sign is a sensitive indicator for assessing hypocalcemia. Although airway assessment is important, the neurological symptoms in this scenario point to an electrolyte imbalance, and establishing the diagnosis is the priority.
