Oral potassium chloride is highly irritating to the gastrointestinal mucosa and can readily cause ulceration or bleeding. To reduce this risk, the medication must be taken in a diluted form and administered with or after meals to minimize gastric irritation. The nurse should ensure that the client understands the dosage form: extended-release preparations must never be crushed or chewed, since doing so would cause an immediate release of the entire dose and result in local mucosal injury.
