For a neutropenic client, any fever (temperature >38.0°C or 38.3°C) is considered an oncologic emergency (neutropenic fever). Because the client's immunity is severely compromised, fever may be the only sign of underlying sepsis, and blood cultures must be obtained immediately along with empirical broad-spectrum antibiotics. Other options such as fatigue or a small plaque should be monitored but are not as life-threatening as fever.
