Warfarin 的治療目標是讓 INR 維持在 2.0-3.0 左右。INR > 4.5 代表抗凝過度（高出血風險）；INR < 1.2 代表抗凝不足（血栓風險）。PT 值隨 INR 波動，通常對應延長。血小板 150k 是正常範圍。
Warfarin therapy aims for an INR of 2.0 to 3.0; values outside this range indicate either excessive anticoagulation with high bleeding risk (INR 4.5) or inadequate protection against clots (INR 1.2). Consequently, both extreme INR values and the corresponding prolonged PT are concerning findings. Normal platelet counts and baseline PT levels are not indicative of therapeutic issues.
