Assistive personnel can perform routine, stable tasks such as assisting with oral hygiene, turning and repositioning the client, and reporting observed skin changes. However, assessment, diagnosis, planning, and evaluation remain the exclusive responsibilities of the licensed nurse, making swallowing assessments and feeding high-risk stroke patients inappropriate for delegation.
