When a vesicant chemotherapy agent extravasates, it can cause severe necrosis and damage to the surrounding tissue. If the nurse suspects extravasation, the first and most critical action is to stop the infusion immediately in order to prevent further medication from entering the subcutaneous tissue. The IV catheter should then be left in place to attempt to aspirate residual drug before removal, and either cold or warm compresses and the appropriate antidote should be used based on the specific drug.
