Digoxin has a negative chronotropic effect and can readily cause bradycardia. The apical pulse must be measured for one full minute prior to administration; if it is less than 60 beats/min in an adult, the drug must be withheld and the provider notified, to prevent serious dysrhythmias or toxicity. Although other vital signs are important, the heart rate is the most critical assessment parameter for directly monitoring this drug's toxicity.
