When administering subcutaneous heparin, the nurse should inject into the abdomen and rotate sites to prevent lipodystrophy and tissue damage. Aspiration is contraindicated as it may cause tissue trauma and bruising, and massaging the site should be avoided to prevent rapid absorption and hematoma formation. Routine PTT monitoring is not required for prophylactic subcutaneous heparin administration.
